About the MHACF: Established in 1946, the Mental Health Association of Central Florida, Inc. (MHACF.org) is one of the oldest nonprofits in Central Florida and is the leading community-based nonprofit dedicated to addressing the needs of those living with mental illness and promoting the overall mental health of all. About the Outlook Clinic: The MHACF Outlook Clinic is guided by the B4Stage4 philosophy, that mental health conditions should be treated long before they reach the most critical points in the disease process. When we think about diseases like cancer or heart disease we start before Stage 4—we begin with prevention, identify symptoms, and develop a plan of action to stop and hopefully reverse the progression of the disease. Like other diseases, it is critical to address symptoms of mental illness early and help the patient plan an appropriate course of action on a path towards overall wellness.
Since 2010 the MHACF has operated Central Florida’s only free and charitable mental health clinic, the Outlook Clinic in downtown Orlando. The program model of providing psychological and medication psychiatric care to the medically underserved and uninsured operates by unique collaborations that bring essential behavioral health care and case management to any person regardless of faith, gender, ethnicity, or ability to provide proof of documentation. Current admissions criteria are patients be 1)11 years+ ) at or below 200% of Federal poverty guidelines 3) not eligible for any governmental health care entitlements or 4) have no healthcare insurance.
In 2022-23 2760 patients were provided care and treatment.
Position Summary: The Youth and Family Program Care Coordinator is a key role responsible for providing case management within the MHACF’s Outlook Clinic Adolescent Program. This individual will provide strategic coordination of care, to ensure the effective delivery of program services. The Care Manager will collaborate with various departments at the Outlook clinic and across the organization as well as with family members and various community collaborators and partners.
Qualifications:
The candidate will possess superior interpersonal skills and the ability to deliver exceptional customer service to a diverse group of constituents from a wide variety of cultural, ethnic, and socioeconomic backgrounds and lifestyles. The Youth and Family Program Care Coordinator will operate in a manner consistent with strength-based, family-centered, and culturally competent values, offers consultation and education to all clients regarding the values of the model, monitors progress toward treatment goals, and assures that all necessary data for evaluation is gathered and recorded.
Education/Experience Qualifications:
- Minimum of bachelor’s degree in social work or related behavioral health field required.
- Certified in QPR (Question, Persuade and Response)
- Excellent communication skills – interpersonal, verbal, presentation and written (also with ability to meet required standards of documentation).
- Must have insight into the Central Florida community behavioral health system of care through professional or personal experience.
- Must demonstrate a high degree of organization and time management skills with strong attention to details.
- Assists with program development as well as ensures compliance with policies and procedures directly related to risk and funding mandates.
- Liaison with appropriate mental health organizations, community agencies and private practitioners.
- Proficiency with Microsoft Office, including Word, Excel and Powerpoint
- Proficiency with Electronic Health Records
- Other administrative duties as assigned.
- Must be enthusiastic for the mission of the MHACF and the clients we serve.
Essential Duties and Responsibilities:
- Maintaining client records, including documentation of services received and case notes
- Coordinating and facilitating the provision of services to clients by other specialists within the organization or community
- Working with clients and their families to develop plans to address specific concerns or needs, such as medical care, housing, child care, or substance abuse treatment
- Conducting assessments for determining eligibility for services and providing information about available resources to clients and their families
- Coordinating case activities, such as arranging meetings with clients, legal staff, social workers, and others involved in each case
- Developing budgets and overseeing expenditures for each case to ensure that funds are being used effectively
- Scheduling appointments and keeping detailed records of progress made towards achieving goals in each case
- Coordinating and maintaining communication between parties involved in a case, including attorneys, social workers, supervisors, and other staff members
- Interacting with other staff members to ensure that cases are handled efficiently and effectively
Collaboration:
- Collaborate with medical professionals, social workers, mental health counselors, school-based personnel, legal advisors, community partners and other relevant stakeholders to ensure comprehensive and holistic care management services.
- Assist with client and family intake and admission processes.
- Build rapport with adolescents, family members and other community partners involved in supporting the client.
- Foster positive relationships with external organizations, agencies, and community resources to enhance service options for clients.
- Connect patients to various resources such as transportation assistance and translation services, as needed.
- Assist families with enrollment into insurance programs.
- Conduct post- service follow up to confirm connection to supportive services in community after client is no longer eligible to participate in adolescent program.
Quality Assurance:
- Implement quality improvement initiatives to enhance the effectiveness and efficiency of case management services.
Data Analysis and Reporting:
- Analyze case management data and generate regular reports to track key performance indicators, identify trends, and inform decision-making.
- Use data-driven insights to assist in making informed decisions that improve overall service quality.
Working Conditions:
This work is performed primarily in an office environment while sitting at a computer terminal for extended periods of time. May involve periodic lifting and carrying of items that may weigh up to 30 pounds. May work outside normal working hours and drive own vehicle for various projects.
