POSITION: Help Me Grow Care Coordinator II 

REPORTS TO: Early Childhood Programs Director 

Wage & Hour Status: Exempt / Full-time  

Hiring Range: $47,424.00 – $52,166.40 

 

PURPOSE: To provide service coordination for eligible children and their families by promoting Help Me Grow to medical providers, families and early childhood stakeholders.


DUTIES AND RESPONSIBILITIES: 

o Oversee the Help Me Grow program and all related activities, including implementation of program, related reporting, case management, and expansion of services. 

o Work closely with the HMG Care Coordinator I and act as their lead for mastery of the HMG system and associated programming. 

o Coordinate all aspects of Kaleidoscope Play & Learn (KPL), including scheduling, attendance, reporting, and implementation to model fidelity. 

o Develop, strengthen, and coordinate collaborative relationships with appropriate organizations and participate in various community meetings and outreach activities as appropriate. 

o Engage and work with Physician Champions to educate and motivate physicians and other child health care providers to conduct child development screenings. 

o Manage the timely response rate to calls and on-line referrals from parents, child care providers/educators, healthcare professionals, and community partners regarding child development topics, concerns, and services offered. 

o Actively work to engage new families/clientele/community and medical partners monthly per HMG Florida requirements.

o Assist in the completion of child development screenings using tools approved by HMG Florida. 

o Use the STAR System, Brookes, other available data, and relevant Help Me Grow/ELCMC systems to access information, resources, document calls, collect required data and produce reports as required.

o Facilitate and oversee the timely implementation of services needed by families and appropriate supports including referrals to community partners to secure additional services based on screening outcomes. 

o Maintain all records confidentially and within ELCMC and Help Me Grow program standards. 

o Work directly with Community Relations staff to develop outreach campaigns and unique advertisement opportunities for the HMG System. 

o Develop, coordinate, and implement Books, Balls and Blocks screening events as required throughout each program year. 

o Work with Early Childhood Program Director to develop annual HMG Work Plan, Continuous Quality Improvement Plan, and Sustainability Plan. 

o Report to ELCMC leadership team to help identify, document, and resolve service gaps and barriers.

o Contribute to a harmonious work environment and support staff in the promotion of ELCMC strategies and goals. 

o Ensure that all information is held in a secure and confidential manner.

o Attend, and assist with, a minimum of three community outreach events annually outside regularly scheduled operating hours of the Coalition, including evenings and weekends. 

o Attend a minimum of two professional development trainings annually as approved by your department officer. 

o Other duties as assigned.

MINIMUM QUALIFICATIONS: 

Education and Experience: 

o Bachelor’s Degree in special education, early childhood development, or related health or human services field. 

o One-year experience working with children and families. 

PREFERRED QUALIFICATIONS: 

Education and Experience: 

o Master’s degree in special education, early childhood development, or related health or human services field. 

o Three years’ experience working in early childhood evidenced-based programming. 

o Three years’ experience collecting, analyzing, and presenting data in order to guide program decision-making. 

o Advanced computer literacy, including knowledge of spreadsheet and database software. 

o Ability to speak, interpret, and translate Spanish and/or other languages

Requirements: 

o While performing the duties of this job, the employee is regularly required to stand; walk; sit for extensive periods of time; use hands and fingers to operate a computer and telephone keyboard and calculator; reach with hands and arms; and stoop, kneel or crouch. In addition, the employee will occasionally lift and/or move up to 50 lbs. 

o Must submit to and pass a local and national background screening, including fingerprinting. 

o Requires a valid Florida Driver’s License, good driving record and automobile insurance. 

o Must have reliable transportation. 

o Must submit and pass a drug screening. 

 o Ability to work flexible hours/schedule, i.e., occasional Saturdays and evening
