POSITION
Grace Medical Home is seeking a dynamic and relational Community Engagement Coordinator to serve as a visible ambassador of our mission. This role is responsible for building strategic community partnerships, coordinating outreach initiatives, and sharing Grace’s story to expand access to care. The position works collaboratively across departments and with community stakeholders to strengthen referral pathways, increase awareness, and support volunteer and provider engagement.


PRIMARY RESPONSIBILITIES

Lead Social Media efforts and create and support communications content, including e-newsletters, digital storytelling, and multimedia messaging

Stay informed on best practices in community engagement and nonprofit communications

Represent Grace Medical Home in the community by building relationships with organizations, schools, nonprofits, civic groups, and local agencies

Develop and strengthen partnerships to increase awareness, referrals, and patient enrollment; distribute outreach materials and promote Discover Grace tours

Plan, coordinate, and execute outreach events (health fairs, church mission fairs, enrollment events), managing logistics, materials, volunteers, and follow-up

Track outreach activities and outcomes; assist with reporting and partner communications

Support volunteer recruitment, engagement, recognition, and appreciation initiatives

Assist with provider and referral specialist recruitment and stewardship, including onboarding/offboarding and database management (Volgistics, Raiser’s Edge, etc.)

Collaborate with leadership to identify specialty care gaps and support provider recruitment efforts


QUALIFICATIONS

Bachelor’s degree in communications, Marketing, Public Relations, Journalism, Community Health, or related field (or equivalent experience)

Minimum of 2 years of experience in outreach, communications, partnership development, event planning, or nonprofit engagement

Demonstrated ability to build and sustain strong relationships with community partners and stakeholders

Strong writing, storytelling, and verbal communication skills

Experience planning and managing events and outreach initiatives

Familiarity with Canva, Adobe Creative Suite, social media platforms, and email marketing tools (e.g., Raiser’s Edge, Constant Contact, Mailchimp)

Highly organized with the ability to manage multiple projects and deadlines

Passion for mission-driven work and serving the community
