Minimum Education Level Required

Master’s degree in nursing with a concentration in an NP specialty from an accredited program. Active unrestricted RN license required.

Minimum Experience Level Required

Minimum three years of direct clinical experience practicing adult primary care in a community or public health setting.

Knowledge, Skills and Abilities

• Advanced knowledge of adult primary care, including pathophysiology, pharmacology, diagnostic testing, and evidence-based management of common acute and chronic conditions.
• Understanding of primary care guidelines, population health and quality measures, and principles of culturally responsive, trauma-informed care in underserved communities.
• Familiarity with community/public health program standards and interprofessional specialty care models.
• Working knowledge of clinic workflows, EHR systems, HIPAA and relevant federal/state regulations impacting nurse practitioner practice and community health centers.
• Strong counseling, health education, and motivational interviewing skills to support behavior change, medication adherence, and risk-reduction.
• Exceptional written and verbal communication skills with the ability to communicate effectively with all levels of the organization and adapt messaging for the appropriate audience; Bilingual preferred.
• Ability to function in a complex team environment, collaborating with external and internal professionals and other customers.
• Self-directed and self-motivated, demonstrated ability to meet deadlines and manage competing demands efficiently and simultaneously.
• Computer skills (word processing, spreadsheets, databases) and effective use of EHR technology and clinical information systems necessary for panel management, documentation and quality metrics tracking.  Familiarity with eClinicalWorks platform preferred.
• Ability to apply evidence-informed guidelines, critically evaluate new information, and adapt practice to emerging care standards and clinical protocols.
• Knowledge of state and federal laws and regulations regarding medical practice and medication dispensing.
• Exceptional time management, decision-making and organizational skills and abilities.
• Strong interpersonal relationship skills, professionalism and high emotional intelligence, with the ability to build non-judgmental, trusting relationships with diverse patient populations.
• Ability to ensure a high level of customer satisfaction including employees, patients, physicians and stakeholders.

Frequent computer use at a workstation up to two hours or more at a time.

Frequently fast-paced work with unscheduled interruptions.

Work may require an adjusted work schedule, overtime, and evening/weekend hours in order to meet patient care demands.
